How to Get Started with Services

To begin, click the New Client Registration link below and complete the registration form. Once your form is submitted, our team will be notified and will contact you within 24–48 business hours to obtain your insurance information and any additional details needed.

After registering, you will receive an email from our practice with next steps and login instructions for the client portal. If you do not see this message, please check your spam or junk folder.

Once you log into the client portal, all required intake forms will load automatically. These forms must be completed in order to move forward.

After registering, you may schedule your first appointment by selecting a therapist or a psychiatric nurse provider (for medication management) from the drop-down menu. Some providers may not appear in the list due to full schedules, but they may still have limited availability for new clients. If you have a specific provider in mind, please contact our office and we’ll be happy to check availability.

All forms must be completed prior to your appointment. If you need assistance completing forms, please contact the office to let us know, and your provider can assist you during your first session if needed.
